Skull of Quakes

In the catacombs, enter the door beside the sign that says 'Towards fate do you travel' using the key of the scion. Follow the cave around to five levers. Figure out the lever puzzle. It seems to change... Save your game first.

Unlock the doors to the north. Head up that way, and go east. The chest is trapped. Inside is the skull of quakes.

Now, return to the upper catacombs. Go to where you fell through the floor to get to the Necromancers' Test. Head generally south until you find the scene pictured above. Note the glowing red slot in the little niche. Put the skull of quakes there. The floor will reconfigure itself, allowing you access to the catacombs teleport pad below.
